How Much Does a BMAC Injection Cost in India?

A lot of patients come to our OPD with a WhatsApp forward or a YouTube video about “stem cell injection for knee” and one simple question: Doctor, isme kitna kharcha hoga, aur kya ye sach mein kaam karta hai? Both are fair questions, and you deserve straight answers to both.

So here is the short version. In India, a BMAC injection for one joint usually costs somewhere between Rs 1 lakh and Rs 3.5 lakh, depending on the hospital, the kit used and whether it is combined with other procedures. Treating both knees can take the bill to Rs 4 to Rs 5 lakh in some centres.

But cost is only half the story. In January 2026, the Government of India released national guidelines on stem cell therapy for bone and joint conditions, and what they say matters a lot before you spend that kind of money. In this article, Dr. Ekta Gupta, Pain Management Specialist (MBBS, MD, Fellowship Trained, AIIMS Delhi), explains what BMAC is, what goes into its price, what the latest evidence says, and which options may give you better value for your knee, shoulder or back pain.

What Is BMAC and Why Is It So Expensive?

BMAC stands for Bone Marrow Aspirate Concentrate. The doctor draws a small amount of bone marrow, usually from the back of the hip bone (the iliac crest), under local anaesthesia. This marrow is spun in a centrifuge to concentrate the cells and growth factors, and the concentrate is then injected into the damaged joint or tendon. Because it contains a small number of mesenchymal stem cells, BMAC is often marketed as “stem cell therapy”.

Compared to PRP, which needs only a simple blood draw, BMAC is a bigger procedure. That is why it costs several times more. A typical BMAC quote covers:

  1. Bone marrow harvest. A special needle is used to draw 30 to 120 ml of marrow from the hip bone. This needs a sterile procedure room and often light sedation.
  2. Processing kit. Branded BMAC concentration kits are imported and are the single biggest part of the bill.
  3. Centrifuge and lab time. The marrow is processed on the same day, often while you wait.
  4. Image-guided injection. Ultrasound or fluoroscopy is used to place the concentrate in the joint.
  5. Procedure room or OT charges. Many centres do BMAC in a day-care OT, which adds facility fees.
  6. Doctor’s fee and follow-up. Including post-procedure reviews and sometimes physiotherapy.

BMAC vs Bone Marrow Aspiration Test: Don’t Get Confused

If you search online, you will also see “bone marrow aspiration cost” listed at Rs 4,000 to Rs 8,000. That is a completely different thing. It is a diagnostic test used by haematologists to check for blood disorders and cancers, and it is usually covered by insurance. BMAC injection for joints is a treatment, costs far more, and is generally not covered.

BMAC Cost in India by Condition

These are approximate ranges seen at private hospitals and regenerative clinics in Indian metros in 2026. Prices change a lot from centre to centre, so use this only to judge whether a quote is in the normal range.

Area treated Approx. cost
One knee Rs 1,00,000 to Rs 3,00,000
Both knees Rs 1,80,000 to Rs 5,00,000
Hip Rs 1,50,000 to Rs 3,50,000
Shoulder (rotator cuff) Rs 1,00,000 to Rs 2,50,000
Tendon (Achilles, elbow, patellar) Rs 80,000 to Rs 2,00,000
Spine (disc or facet) Rs 1,50,000 to Rs 3,50,000

Knee arthritis is by far the most common reason people are offered BMAC. Some centres sell it as a way to “regrow cartilage” and avoid knee replacement. As we explain below, the evidence does not support that promise yet.

BMAC Cost by Procedure Type

The word “BMAC” is used for a few different procedures, and the price changes with each.

Procedure type What it involves Approx. cost
BMAC injection alone Marrow harvest, same-day concentration, guided injection into one joint Rs 1,00,000 to Rs 2,50,000
BMAC + PRP combination BMAC mixed with PRP from a blood draw, sold as a package Rs 1,50,000 to Rs 3,50,000
BMAC during surgery Added during arthroscopy, cartilage repair or core decompression for hip AVN Adds Rs 50,000 to Rs 1,50,000 to the surgery bill
Culture-expanded stem cells Cells grown in a lab over weeks before injection Higher, and legally allowed only in approved clinical trials

The last one is important. Cells that are grown and multiplied in a lab count as “more than minimally manipulated” and need prior approval from the drug regulator CDSCO, plus an ethics committee, even for research use. If a clinic offers lab-grown stem cells for your knee as a routine paid treatment, that is a serious red flag.

What Does the Government of India Say About BMAC?

This is the part most BMAC pages online will not tell you. In January 2026, the Ministry of Health and Family Welfare, through the Department of Health Research and DGHS, published Evidence-based Guidelines for the use of Stem Cell Therapy in Orthopedic Conditions. The expert group reviewed randomised trials that included bone marrow based treatments like BMAC.

Their conclusion was clear. Stem cell therapy is not recommended in routine clinical practice for:

  • Osteoarthritis (including knee arthritis)
  • Cartilage defects
  • Tendinopathy (tennis elbow, rotator cuff, Achilles, patellar tendon)
  • Avascular necrosis of the hip
  • Non-union of bone
  • Meniscal tears

For each of these, the guideline says the treatment may be used only inside properly conducted randomised controlled trials with ethics committee approval. For knee osteoarthritis, the panel found that the pain relief at 12 months was too small to make a real difference for patients, and the certainty of the evidence was very low. They also noted the large costs involved and felt that standard care was probably more cost-effective.

In plain words: if you are paying Rs 2 or Rs 3 lakh out of your own pocket for BMAC as a regular treatment, you are paying for something the Government’s own experts say is not yet proven. Our honest advice is to ask any centre offering it whether you are being enrolled in an approved trial, and to see the ethics approval in writing.

BMAC vs PRP vs Other Options: Cost and Evidence

If your goal is to manage knee pain and delay surgery, here is how BMAC compares with other options, using approximate costs per knee.

Treatment Approx. cost Where the evidence stands
Physiotherapy and weight management Rs 5,000 to Rs 20,000 Strongest evidence, first step for everyone
Steroid injection Rs 1,500 to Rs 5,000 Short-term relief, a few weeks to months
PRP injection Rs 8,000 to Rs 25,000 per session Good evidence in mild to moderate knee arthritis
Genicular nerve radiofrequency ablation Rs 35,000 to Rs 80,000 Good option for pain relief in moderate to severe arthritis
BMAC injection Rs 1,00,000 to Rs 3,00,000 Not recommended for routine use as per 2026 Indian guidelines
Total knee replacement Rs 2,00,000 to Rs 5,00,000 Definitive treatment for end-stage arthritis

Notice something? PRP, which costs a fraction of BMAC, has better support for knee arthritis. So far, studies comparing the two head to head have not consistently shown BMAC to be better. Our detailed guide on PRP injection cost in India and our blog on interventional pain management for knee osteoarthritis cover these options in more depth.

Hidden Costs and Questions to Ask

If you are still considering BMAC, ask for a written, all-inclusive quote. Check whether it includes:

  • Consultation and pre-procedure tests (CBC, coagulation profile, viral markers)
  • MRI, if you don’t have a recent one
  • OT or day-care charges and sedation
  • The concentration kit and any PRP add-on
  • Follow-up visits and physiotherapy

And ask these questions directly: How many cells will be injected, and will you show me the cell count? Is this part of an ethics-approved trial? What happens if I see no improvement in six months?

Questions to ask before paying for BMAC injection

Is BMAC Covered by Insurance in India?

Almost never. Since BMAC is not recommended for routine use, health insurers in India treat it as an experimental procedure and do not reimburse it. You will need to pay the full amount yourself. Some hospitals offer EMI options, but please be careful about taking a loan for a treatment with unproven results.

Who Might Still Consider BMAC?

BMAC is not useless. Researchers are still studying it, and there are situations where a surgeon may use bone marrow concentrate as part of a larger operation, such as core decompression for early hip AVN or certain cartilage repair surgeries. The right setting for BMAC today is:

  • An approved clinical trial at a recognised institution, with ethics committee approval
  • As an add-on during surgery, decided by an orthopaedic surgeon after explaining the evidence

It is generally not the right choice if you have bone-on-bone arthritis, active infection, blood disorders, cancer, uncontrolled diabetes, or if you are on blood thinners that cannot be stopped.

What Happens During a BMAC Procedure?

So that you know what you are signing up for, here is how it usually goes. The procedure takes about one to two hours. You lie on your stomach, the skin over the back of the hip is numbed, and a thick needle is used to draw marrow from the pelvic bone. Some centres add light sedation. The marrow is processed for 20 to 30 minutes and then injected into the joint under imaging.

Most people go home the same day. Expect soreness at the hip for three to seven days, and some swelling in the treated joint. Painkillers like ibuprofen are usually avoided for a while. Any improvement, if it comes, takes two to six months to show.

Frequently Asked Questions

What is the cost of a BMAC injection in India?

A BMAC injection for one joint usually costs between Rs 1 lakh and Rs 3.5 lakh in India. Treating both knees can cost up to Rs 5 lakh in some hospitals. The price depends on the kit, the hospital and whether PRP or surgery is added.

Is BMAC the same as stem cell therapy?

BMAC contains a small number of stem cells from your own bone marrow, so it is often called stem cell therapy. It is different from lab-grown stem cells, which are multiplied for weeks before injection and need regulatory approval.

Is BMAC approved in India?

The 2026 national guidelines from the Ministry of Health and Family Welfare do not recommend stem cell therapy, including BMAC, in routine clinical practice for osteoarthritis, tendon problems, cartilage defects and similar orthopaedic conditions. It may be used within ethics-approved clinical trials.

Is BMAC better than PRP for knee pain?

Current evidence does not show that BMAC is clearly better than PRP for knee arthritis, even though it costs many times more. For most people with mild to moderate arthritis, PRP or other proven options are the better first choice.

Can BMAC regrow knee cartilage?

This is often claimed but has not been proven in good quality trials. Be careful of any clinic that promises cartilage regrowth or guarantees you will avoid knee replacement.

Does insurance cover BMAC?

No. Health insurance companies in India generally treat BMAC as experimental, so you will have to pay the full cost yourself.

Is the BMAC procedure painful?

It is done under local anaesthesia, sometimes with mild sedation. Most people feel pressure during the marrow draw and soreness at the hip for a few days afterwards.

Final Word

BMAC injection cost in India runs into lakhs, and right now the Government’s own expert panel says the benefit is not proven enough for routine use. That does not mean your knee or shoulder pain has no solution. It means the smarter path is to get a proper diagnosis and start with treatments that have real evidence behind them, many of which cost far less. If you have been offered BMAC or stem cell injections and are unsure what to do, speak to Dr. Ekta Gupta in Rajouri Garden, Delhi, for a clear, honest opinion.
